VA Community Care Network Program

The Hyndman Area Health Center participates in the VA Community Care Network. We understand that transportation to and from VA facilities may be a challenge. You can contact you VA provider to be seen at one of our facilities.

With an authorization from VA Community Care Network, you may be able to see our providers for your VA related issues as well as your primary care needs.

 

How Do I Schedule an Appointment?

  1. To schedule an appointment, contact your registered VA Medical Center and explain to the operator, “I would like to obtain medical services outside of the VA system.” Search for your VA by going to: www.va.gov/directory/guide/home.asp
  2. Also, let them know which services you need (medical or dental). Your VA representative will explain your eligibility over the phone.
  3. If you are approved, the VA will work with the Hyndman Area Health Center to schedule you an appointment on your behalf! Prior to receiving service at one of our facilities, you we will need an authorization from the VA Community Care Network.

This health center receives HHS funding and has federal PHS deemed status with respect to certain health or health-related claims including medical malpractice claims for itself and its covered individuals.
