Patient Information

The Hyndman Area Health Center believes in Patient First Service. At each of our three facilities we work with patients to help them to achieve their health care goals.

Call us at Hyndman, Bedford, Richland, Everett to become a new patient today or to schedule an appointment. Click the links below to access our patient documents. You will be connected to one of our Patient Service Representatives who will be happy to assist you. The Patient Service Representative will collect demographic information, and will help you schedule your first appointment.

 

Your First Appointment

When you come for your first visit, please arrive at least 15 minutes ahead of your appointment, so you have time to register and complete required paperwork. If you want to fill out the forms ahead of time, click on the links below to print or complete and email back to us. For every appointment you should bring your photo ID, insurance card(s), a list of your current medications, and any co-pay required by your insurance. Paying for Your Health Care

We accept all insurances, including Medicaid, Medicare and CHIP. We accept commercial insurances you get through work or associations, or through the Federal Insurance Marketplace. If you do not have insurance, we will be happy to meet with you to assist you with obtaining insurance. Please call us at (814) 842-3206 and we will be happy to assist you.

No matter what your financial situation is, you can still receive care at Hyndman Area Health Center. No one will be denied health care because they are unable to pay. We have a sliding fee discount program based on income and family size. We also have a discount prescription program. You can click here to review our financial services.

 

After-Hours Information

For established patients, please call the following number: 800-500-6747 if you have an emergency. We will try to accommodate and offer after hour services. Our answering service will then contact the doctor on call and the doctor will then return your call. We are unable to offer this service for patients who have not yet been seen by our providers.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.

We offer telemedicine to all of our patients. On the patient portal you can access several resources including:

  • Viewing upcoming appointments
  • Sending messages to your provider
  • View billing statements
  • Request appointments and labs
  • View medical history.

Forgoing your healthcare will have a negative impact on your health that may lead to urgent care and/or hospitalization setting. Our primary care providers are here to keep the patient well with medication management, control and/or prevent chronic diseases and treat acute illnesses. It is very important to establish a strong Patient-Provider relationship to ensure the best clinical outcome. The cost of an urgent care visit is typically two times the cost of a primary care visit. The cost of an emergency room visit is typically nine times the cost of a primary care visit.
