Chronic Care Management is designed to empower individuals with chronic diseases to better manage their care and improve their quality of life, through the assistance of a care manager and connected care. Connected Care is the real-time communication between the patient and the care manager or provider via phone, electronic communication, etc. Connected care provides personalized teaching to help patients track and manage their chronic condition at home.

Our RN Care Managers are available to provide services including, but not limited to:

• Condition education (diabetes, high blood pressure, high cholesterol, etc), including dietary modifications and other lifestyle adjustments.
• Medication education
• Assist with coordination between specialists
• Provide community resources
